ABSTRACT

Objective To explore the feasibility and safety of carotid artery stenting (CAS) via right transradial approach (TRA).Methods A retrospective analysis was made on 46 cases undergoing the CAS via right TRA.Patients were divided into the right carotid artery group (RCA),bovine left carotid artery group (B-LCA),nonbovine carotid artery group (NB-LCA) according to the lesion location.Low TRA or high TRA were selected to overcome the difficulties of insufficient supporting power, using techniques such as catheter looping and retrograde engagement technique(CLRET).Results CAS were successful in all cases (100%),the differences of the operation time and fluoroscopy time among RCA group,B-LCA group and NB-LCA group were not significant. In NB-LCA group,CLRET were applied in 10 cases (55.56%,10/18),includeing 8 cases with type Ⅲ aortic arch(100%,8/8)and 2 cases with type Ⅱ aortic arch (33.33%,2/6).The CLRET prolonged the operation time and fluoroscopy time [(39.45 ±7.27) min vs.(30.80±4.66) min;(11.84 ± 2.05) min vs.(9.91 ± 1.45) min,P <0.05).There was no cerebrovascular events and puncture point complications in perioperative period. Conclusion Right transradial approach for carotid artery stenting is safe and technically feasible, especially in RCA stenosis and LCA stenosis with type Ⅰ or Ⅱ aortic arch.

ABSTRACT

Objective To study comprehensive nursing meassures for small area of diabetic foot ulcer. Methods A total of 58 patients with small area of diabetic foot ulcer in our department were divided into two groups. The comprehensive nursing group (CN group,30 cases) adopted comprehensive nursing and treatment as follows, surgical dcbridement, infrared radiation of wound surface, sprinkling of growth factors(bFGF and rhEGF),covered by hydrocolloid dressing(Algoplaque). The control group(28cases) received hydropathic compress with 0.5% povidone iodine or 5% furacillin solution.Wound heal-ing progreasion, average healing time and the positive rate of wound germiculture of the two groups were observed. Results The average healing time of the CN group was (29.6±9.4)d,which was shorter than that of the control group,( 39.2±13.5 )d, P < 0.05. Statistical difference existed in the positive rate of wound germiculture at the 7th day and the 14th day between the two groups (P < 0.05). Conclusions Routine medical treatment and general nursing measures plus surgical debridement, infrared radiation of wound sur-face,sprinkling of growth factors and hydrocolloid dressing possess facilitative healing effect for small area of diabetic foot ulcer.

ABSTRACT

Objective To establish nursing intervention for patients with diabetes mellitus and discuss in-fluence of community nursing on rehabilitation of patients with diabetes mellitus. Methods Patients with con-finned diagnosis of type two diabetes mellitus (61 cases) were selected, individual health records were established and health education was given to them, including diet, exercises, psychological nursing, medication instruction and regular blood glucose monitoring. Patients compliance, control of fasting blood glucose and glycosylated hemoglobin were compared before and after nursing intervention. Results Patients compliance such as regular physical ex-amination,diet control, regular blood glucose monitoring and exercises treatment improved after intervention com-pared with those before community intervention (P<0.05). The control of fasting blood glucose and glycosylated hemoglobin was better after intervention compared with those before community intervention (P<0.01). Con-clusion Community nursing intervention for patients with diabetes mellitus proved to have good effect.
